Application Guidelines

Applications are reviewed for accuracy, completeness and strong plans for sustainability beyond the initial award.

A letter of support must be submitted with your application. This letter must clearly state your senior leadership's commitment, financially and otherwise, to sustain the Reach Out and Read Program long term. The support letter must be signed by the Medical Director and/or Executive Director. Applications are not complete and will not be reviewed until this letter is received.

Site Contact

All contact and site information must be complete. Please fill in the blanks accordingly. An email address is mandatory for the primary mailing contact.

Satellite Sites

Satellite sites are those sites that do not submit an individual application to the ROR National Center, but are practicing the ROR program model in connection to an "official" program site. Please complete the ROR Satellite Site Information Form for each site. Although multiple sites fall under a single hospital or medical system umbrella, they can still be considered distinct ROR programs, and thus each site is strongly encouraged to apply for its own funding.

Demographics

The application must accurately reflect the number of participating medical providers.

Questions asking for Insurance Coverage and Patient Annual Income, as a percent of the Federal Poverty level, must be answered in percentages totaling 100%. Medical institutions that serve a low -income population are currently eligible for funding from the ROR National Center.

Annual Book Budget

Your site's annual book budget must reflect the accurate number of well child visits for children between the ages of six months and five years only. Sick and follow-up visits should not be included.

Fundraising

All awards are restricted to book purchases only and cover a portion of the first year's book budget. Therefore, your application must clearly outline a specific plan for obtaining on-going funding for your site beyond the start-up grant. Funding requests for anything other than books (i.e., staff time, furniture, etc) will not be considered in the review process.

Volunteers/Literacy Rich Waiting Area

Plans for recruiting and training volunteer readers should also be outlined in this section. As well as any plans to create a literacy rich environment in your sites waiting area.
