Applications Guidelines
- Applications are reviewed for accuracy and thoroughness, commitment to the Reach Out and Read model, and strong plans for sustainability. All clinics/practices applying to start Reach Out and Read Programs are required to demonstrate that they have secured at least 75% of their Annual Book Commitment (ABC, the total number of books needed for a Reach Out and Read Program annually, which is equivalent to the annual number of well-child visits for children ages 6 months through 5 years).
- A letter of support must be submitted with your application. This letter must clearly state your senior leadership's commitment, financially and otherwise, to sustain the Reach Out and Read Program long-term. The support letter must be signed by the Medical Director and/or Executive Director. Applications are not complete and will not be reviewed until this letter is received.
Program Contact
- All contact information must be complete. Please fill in the blanks accordingly. An email address is mandatory for the primary mailing contact.
Satellite Programs
- Satellites are clinical locations/primary care locations that do not submit an individual application to the Reach Out and Read National Center, but are practicing the Reach Out and Read model in coordination with an "official" Program. Please complete the Reach Out and Read Satellite Program Information Form for each qualifying Satellite. Although multiple Programs may fall under a single hospital or medical system umbrella, they are still considered distinct Reach Out and Read Programs, and thus each Program is strongly encouraged to apply on its own.
Demographics
- The application must accurately reflect the number of participating medical providers.
- Questions asking for Insurance Coverage and Patient Annual Income, as a percent of the Federal Poverty level, must be answered in percentages totaling 100%.
Annual Book Commitment (ABC)
- Your Program's Annual Book Commitment must reflect the accurate number of well-child visits for children 6 months through 5 years only. Sick and follow-up visits should not be included.
Fundraising
- Your application must clearly outline a specific plan for obtaining ongoing funding for books for your Program.
Volunteer Readers/Literacy-Rich Environment
- Plans for recruiting and training volunteer readers should also be outlined in this section, if applicable, as well as any plans to create a literacy-rich environment at your Program.
